​Referrals​

​

We welcome referrals from GPs, specialists, or other healthcare providers. If someone has referred you to us, please bring along any relevant documents or reports to your appointment.

​
Private Health Insurance

​

If your private health insurance includes cover for Physiotherapy or Exercise Physiology, please bring your health fund card to your appointment. You can claim your rebate on the spot using our HICAPS system.

​

If you're unsure about your coverage or rebate amount, we recommend contacting your health fund directly before your visit.

​

Medicare - GP Chronic Condition Management Plan (GPCCMP)

​

If you have a chronic condition, you may be eligible for Medicare rebates under a GP Chronic Condition Management Plan (GPCCMP).

​

  • A GP referral is required to access this benefit.

  • Eligible clients can receive up to 5 subsidised sessions per calendar year for Physiotherapy or Exercise Physiology.

  • Full payment of $81.80 is required at the time of your appointment.

  • You’ll receive a $61.80 Medicare rebate, processed directly into your bank account via our Medicare patient claim system.

  • This results in an out-of-pocket fee of $20 per appointment.

Please ensure your referral is up to date and bring it with you to your appointment.

Preparing for Your Consultation

What to Bring

​

  • Any relevant GP or specialist referrals

  • Recent x-rays, scans, or imaging reports
    (Ask your GP, specialist, or radiology provider to email or fax these to us in advance, if possible.)

  • A water bottle (especially for exercise or Pilates sessions)

  • Any aids, braces, or exercise programs you currently use or have used previously

​​What to Wear

​

  • Please wear or bring comfortable, loose-fitting clothing.

  • If your concern involves the hip, knee, shoulder, or spine, bringing shorts or a singlet can help with assessment.

  • For gym or Pilates sessions, wear activewear that allows free movement and lets us observe posture and form.

​

Grip socks are required for all Clinical Reformer Pilates sessions for safety reasons. We offer MoveActive grip socks at reception ($22/pair) in a variety of sizes and styles, or feel free to bring your own.
